Transcript Requests

Official transcripts are issued by the Registrar’s office in Madison. If you would like your official UW Colleges transcript sent to another institution, or if you would like a copy for yourself, the request must be submitted in writing (telephone requests are not accepted). There is a $6.00 charge per transcript, and all requests must include a valid signature.

Please download and fill out the Transcript Request Form. Note that you must use Adobe Acrobat (free) or compatible software to view the form.

There are five ways to request an official transcript:

  1. In Person: download and fill out the Transcript Request Form, print and sign it, and pay the fee at the UW-BC Business Office if your campus accepts in-person payment.
  2. Via Mail (Form): download and fill out the Transcript Request Form, print and sign it, and include check or credit card information when mailing it.
  3. Via Fax: download and fill out the Transcript Request Form, print and sign it, and fax it with credit card information.
  4. Via Email: download and fill out the Transcript Request Form, print and sign it, and email it with credit card information.
  5. Via Mail (Letter Request): include your name, social security number, campus attended, dates of attendance, and name and address of the recipient in your request letter. Sign your letter, and mail it with a check.

Mail requests to the following address. Please allow 7-10 working days for the transcript to arrive at its destination.

A printout of your unofficial transcript is available by accessing your PRISM account.

Transcripts will not be furnished for a student who has an outstanding obligation to UW-BC (e.g., owes a fine, money, materials, or financial aid). UW-BC does not issue copies of official transcripts or documents that it has received from other institutions.
